Disciplinary Actions

 

When the Board takes disciplinary action against a licensee, a brief statement of the conduct and the sanctions imposed will appear on the Board’s web page for a period of time equal to the length of the disciplinary action, or one year, whichever is greater.  Licensees who have failed to meet the conditions imposed will remain on the web page until the conditions are met.  NOTE: Disciplinary actions taken by the Board remain on the licensee’s record even though all conditions imposed by the Board have been met.

1/24/2011	Jeron Cordell Hawkins #5722	Violated the May 18, 2009 Order of the Board, failing to provide evidence of completing the six points CCA in addition to the 15 points required to renew his license for 2010 - 2011.	An Order was issued suspending his license from 2/1/11 to 3/31/11 until showing proof to Board of six CCA points, complete jurisprudence Exam, obtain mentor for one year after completion of terms of suspended license and not violate Practice Act and Rules of the Board.  Assessed civil penalty and costs.
5/16/2011	Sherman B. Bibee, Jr. #5196	Falsified time sheets	A Consent Order was issued on 5/16/11.  License put on probation for 2 years.  Assessed civil penalty and disciplinary costs.  Additional CCA points required for 2012 renewal.
7/18/2011	Alan Charles Gorrod #0068	Documented occupational therapy services that were not rendered.	A Consent Order was issued on 6/18/11.  License put on probation for 2 years.  Assessed civil penalty and disciplinary costs.  Complete the jurisprudence exam and additional CCA points for 2012 renewal.
7/18/2011	Nancy E. Hickmon #6318	Documented occupational therapy services that were not rendered.	A Consent Order was issued on 7/18/11.  License put on probation for 2 years. Complete the jurisprudence exam.  Assessed civil penalty and disciplinary costs.
7/18/2011	Reshma Dilipkumar Patel #7879	Practiced without a license from November 2010 through May 12, 2011.	A Consent Order was issued on 7/18/11.  License put on probation for 2 years.  Assessed civil penalty and disciplinary costs.  CCA required for 2012 (first) renewal.
7/18/2011	Angela A. Sigmon #5638	Engaged in inappropriate sexual relationship while OT/patient relationship existed.	License put on probation for 2 years.  Assessed civil penalty and disciplinary costs.  Additional CCA points required for 2012 renewal.  Complete the jurisprudence exam.
3/19/2012	Marilyn Cope #6831	Failed to follow plan of care, physician’s orders.  Failed to review POC, goals, precautions or evaluation prior to treating.	An Order was issued on 3/19/2012.  License put on probation for 2 years.  Assessed civil penalty and disciplinary costs.  Additional CCA points required for 2013 renewal along with successful completion of jurisprudence exam.
7/16/2012	Betsy Christina Smith #6604	Failed to complete continuing competence activity in a timely manner for 3 (three) renewal periods	Letter of Reprimand.  Assessed civil penalty.
7/19/2012	Brian Gorman #6858	Failure to comply with CCA Audit	An Order was issued on 7/19/2012.  Assessed civil penalty and disciplinary costs.  Complete the jurisprudence exam.
7/19/2012	Jessica Gentz #7672	Submitted a NSF check to the Board in payment of her 2012-2013 license renewal. As a result she practiced without a current license from July 1, 2012 – to July 19, 2012.	A Consent Order was issued on 9/17/12. License put on probation for 1 year.  Assessed civil penalty and disciplinary costs.
8/1/2012	Chamar Jackson #6155	Failed to complete continuing competence activity in a timely manner for 3 (three) renewal periods	Letter of Reprimand.  Assessed civil penalty.
9/17/2012	Amanda Dunaway #7620	Tested positive for drug use while at work	A Consent Order was issued on 9/17/12 revoking her license to practice for one year.  Assessed civil penalty and disciplinary costs.  Must provide Board with drug abuse assessment and evaluation.
9/17/2012	Sharon M. Lankes #7199	Renewed her license late resulting in her practicing without a current license from July 1, 2012 – to August 6, 2012.	A Consent Order was issued on 9/17/12. License put on probation for 1 year.  Assessed civil penalty and disciplinary costs.
9/17/2012	Patricia Anne Perkins #7622	Submitted a NSF check to the Board in payment of her 2012-2013 license renewal. As a result, she practiced without a current license from July 12 2012 – to July 25, 2012.	A Consent Order was issued on 9/17/12. License put on probation for 1 year.  Assessed civil penalty and disciplinary costs and required that she complete the jurisprudence exam.
11/12/2012	Jeron Cordell Hawkins #5722	Failed to provide adequate, timely and complete documentation pursuant to contract with employer for October-November 2011 and January 2012.  Failure to do so was a violation of the Practice Act and Rules of the Board.	An Order was issued on 11/12/2012.  Cannot supervise OTA or student, obtain mentor, have supervisor make monthly reports that he is completing documentation, have direct supervision, pay civil and costs and not violate the Practice Act or Rules of the Board during his probation. License suspended  until 5/31/13 and after suspension, on probation for addtional two years.  Assessed civil penalty and disciplinary costs.
11/12/2012	Carolyn Royster #6189	Failed to secure treatment files and allowed files to be in possession of third party for two weeks.	An Order was issued on 11/12/12.  Assessed civil penalty and discipilnary costs.
1/28/2013	Virginia McGinnis #6324	Lack of documentation and billing for OT services not rendered.	A Consent Order was issued on 1/28/13. License put on probation for one year.  Assessed civil penalty and disciplinary costs.  Additional CCA points required for 2013 renewal.
1/28/2013	Denise Hudson Nesbitt #4586	Billing for OT service not rendered and falsified documents/ therapy notes.	A Consent Order was issued on 1/28/13.  License put on probation for two years.  Assessed civil penalty and disciplinary costs.  Complete the jurisprudence exam.
