2008 INSTRUCTIONS FOR COMPLETING RENEWAL APPLICATION
PLEASE READ CAREFULLY!!!
License renewals are due by June 30th each year. Renewal Applications and Continuing Education Cards must both be complete and postmarked by June 30th to be on time. If you do not intend to renew your license, please return the enclosed Notification of Non-renewal Form.
Renewal Applications are mailed to the most recent address on file with the Board. Pursuant to Rule .0401 of the Rules and Regulations, it is the licensee’s responsibility to notify the Board in writing of each change of residence, business, or mailing address, within ten days of such change. You may download a renewal application at www.ncbot.org. The Board will not re-mail or fax the application.
You are encouraged to MAIL EARLY! You may renew any time after April 1. Failure to renew by June 30 will result in expiration of your license. If your completed renewal package is received after June 30, 2008, your license may be eligible for reinstatement upon receipt of your completed renewal application, completed continuing education card, regular $50.00 renewal fee, additional $50.00 late fee and verification that you did not practice after June 30, 2008. Disciplinary action will be taken if you practice after your license has expired. “Non-sufficient funds” checks will be considered late renewals if restitution is not received by June 30th. Due to the high volume of renewals submitted in June it may take up to two weeks to process your renewal application. During this processing period, your license may not be able to be verified online as being current.
Print your name and license number in the memo section of your check to ensure proper credit. If your employer is paying your renewal fee, instruct the payer to include your name and license number in the memo section of the check. Do not submit one check for multiple therapists. The Board does not issue receipts. Verifications will not be provided by telephone. Your employer may verify your renewal online at www.ncbot.org under Verifications. There is a $20.00 charge for written verification.

THE BOARD WILL RETURN PARTIAL APPLICATIONS. Licenses will not be considered renewed until all required and completed documents have been received.

The renewal period begins April 1. You may check the status of your renewal at www.ncbot.org under Verifications. (Be sure to capitalize your last name.) Renewals will not be confirmed by telephone. The Board will make every attempt to respond to telephone calls and questions, however, this is an extremely busy time. Please listen to the phone prompts; your question may be answered within a prompt. You may also e-mail your questions. E-mails will be answered as time permits. Your patience will be appreciated.
OTAs must provide proof of licensure to their supervising OT and have their renewal applications signed by their supervising OT. If you are unemployed, you may indicate so on the application. No OT supervision signature is required if you are not employed as an OTA.

If your renewal information has not been returned to you within 10 business days from the time you mailed your application and your expiration date has not been updated at www.ncbot.org, please notify the Board in writing or by e-mail. You should receive your renewal wallet card within 10 business days after your license has been renewed. PLEASE DO NOT CALL THE BOARD OFFICE TO INQUIRE ABOUT YOUR RENEWAL.

Please note: Licenses not renewed by June 30, 2007 are expired. There is no “inactive status.” Your license is either current or expired. If you choose to renew your license within 24 months of the expiration date, you will still be required to complete the continuing education requirements and pay the renewal fees for the period of time your license was not current, along with any applicable late fee.
Persons whose licenses have expired for more than 24 months, and who desire to reinstate their licenses, must submit a new license application to the Board and meet all requirements then existing.
The OT and OTA Supervision Change Forms may be downloaded at www.ncbot.org. Pursuant to Rule .0401 you must notify the Board, in writing, within 10 days of any changes.
