Mailing Lists

The North Carolina Board of Occupational Therapy offers a mailing list of currently licensed Occupational Therapists and Occupational Therapy Assistants.  The list may be purchased by sending a written request along with a money order, certified check or cashier’s check (no personal checks or company checks accepted) in the amount of $300.00 to the Board address below. The lists may be pre-sorted by name, designation or designation and zip code.  The lists are available in Excel format.  You may download the request form below.
