Mailing Lists

 

The North Carolina Board of Occupational Therapy offers a mailing list of currently licensed Occupational Therapists and Occupational Therapy Assistants.  The list may be purchased by sending the Mailing List Request form (print from link below) along with a money order, certified check or cashier’s check for $300.00 to the Board address below.  

 

The list is sent by Email in Excel format and can be pre-sorted by name, designation or designation and zip code

 

Special Notes: 

    1.        Licensee’s e-mail or telephone information will not be provided.

    2.        Personal or company checks cannot be accepted.
